Complete the Reporting Form provided by the Medical Board of California, linked here. If a midwife has transferred a patient to you, the midwife may have already filled some of this form out for you.
Fax the completed form to the Medical Board of California at (916) 263-8396 and fax page 1 only to CMQCC at (650) 721-5751.

The goal of our organization and our staff is to help facilitate safe, legal, and humanized healthcare, including cooperation and collaboration in the care of a client following a transfer from an out-of-hospital setting. We deeply appreciate providers and hospitals who also facilitate this collaboration.
Some information about Licensed Midwives: Licensed Midwives are autonomous care providers of low-risk people throughout pregnancy, labor, birth, and 6 weeks postpartum. Healthy newborn care to 6 weeks of age also falls within the scope of practice of the Licensed Midwife. Licensed Midwives are neither required by law or training nor expected to have physician supervision.
